1. Introduction
Welcome to Perfect 10.0 Physical Therapy & Performance Training. By accessing our services, you agree to comply with the terms and conditions outlined below. These terms govern your use of our physical therapy and performance training services and our interactions with you.
2. Appointments
- Scheduling: Appointments can be scheduled by calling 512-426-6593. All appointments are subject to availability.
- Rescheduling and Cancellations: Clients must notify us at least 24 hours in advance to reschedule or cancel appointments. Failure to do so may result in a cancellation fee.
- Late Arrivals: If you are late to your appointment, the session may be shortened to accommodate other clients. Full session fees will still apply.
3. Payments and Fees
- Payment Policy: Payments are due at the time of service unless otherwise agreed. We accept cash, credit/debit cards, and other approved payment methods.
- Insurance: Perfect 10.0 Physical Therapy & Performance Training is an out-of-network provider. Clients are responsible for verifying their insurance coverage and submitting claims independently.
- Returned Payments: A $25 fee will be applied to any returned payments or insufficient funds.
4. Liability and Assumption of Risk
- Informed Consent: By engaging in therapy or performance training services, you acknowledge that you have been informed of the nature of the activities and the potential risks involved.
- Limitation of Liability: Perfect 10.0 Physical Therapy & Performance Training and its staff are not responsible for injuries, damages, or losses incurred as a result of non-compliance with instructions, pre-existing conditions, or other unforeseen circumstances.
5. Privacy and Confidentiality
- Personal Information: Your personal and medical information will be kept confidential in compliance with HIPAA regulations.
- Data Sharing: We will not share your information with third parties without your explicit consent unless required by law.
6. Code of Conduct
- Respectful Environment: All clients and staff are expected to maintain a respectful and professional environment. Harassment, abuse, or inappropriate behavior will not be tolerated.
- Compliance: Clients must follow all instructions and safety guidelines provided by staff to ensure effective and safe care.
7. Changes to Terms
Perfect 10.0 Physical Therapy & Performance Training reserves the right to update these terms and conditions at any time. Clients will be notified of significant changes.
8. Governing Law
These terms are governed by the laws of the State of Texas. Any disputes will be resolved under the jurisdiction of the courts located in Travis County, Texas.
